Congratulations to the remaining applicants who are still qualified for enrolment (the waitlist) of the Bachelor of Secondary Education major in Mathematics and Science of the College of Education for 1st Semester of Academic Year 2025 – 2026.
To confirm your slot in the program, please accomplish the following:
- Fill out the confirmation slot in the link on or before May 28, 2025 https://forms.gle/7hQ2s5zCACtjiRoeA
- Fill out the Student Data Sheet in the link on or before May 28, 2025 https://forms.gle/uMyGVX7ECMYG9w5S8
Submit the following initial requirements to the Office of the Dean, College of Education, Talisay Campus at LSAB 114 on or before May 28, 2025
- Original Report Card for senior high school graduates, or transfer credentials with photocopy of Transcripts of Records for transferees.
- Certificate of Good Moral Character.
- One (1) original copy with clear photocopy of NSO/PSA Birth Certificate
- One (1) original copy with clear photocopy of NSO/PSA Marriage Certificate (additional requirement for married female applicant)
- Two (2) pcs recent 2×2 ID picture
- 1 pc long brown envelope with label at the back
Additional documents/requirements may be needed upon enrolment by the University Registrar.
Also, applicants who did not qualify may retrieve their documents after May 23, 2025 at the College of Education office (LSAB 114).
