Capturing of student data for Identification card is on-going
All first-year and transfer students in all campuses must fill out the Online Identification Card Form for their ID processing until September 7, for Academic Year 2024-2025.
You can access the online form at cidsa.chmsu.edu.ph/student
Both male and female students must observe a neat hairstyle and appropriate hair color.
