The College of Education released the list of qualified applicants for enrollment in the Bachelor of Technical Vocational Teacher Education (BTVTED) program at the Alijis Campus.
Requirements for Enrollment (First Year and Transferees:
- Original Report Card for Senior High School Graduates
- Certificate of Rating (COR) for ALS Completers
- Transfer Credentials with a photocopy of the Transcript of Records for Transferees
- Certificate of Good Moral Character
- Admission Slip and/or loading slip
- 2 copies of NSO/PSA Birth Certificate
- 2 copies of NSO/PSA Marriage Certificate for married female applicant
- 2pcs recent 2×2 ID picture
- 1 long white/cream folder
- 1 long brown envelope
- Filled out Student Information Sheet (SIS) to be distributed upon your enrollment
- Filled out Online Student Information Sheet System (OSISS) osiss.chmsu.edu.ph
- Php 130.00 Insurance Fee
