1. Accomplished Clearance Form
2. Accomplished Request to Return to School for Returning students
3. Loading Slip from the Program Chair (for irregular students)
4. School Identification Card
5. Insurance Fee (Php. 130.00) for Returning students
6. Alumni Fee (Php. 300.00) for Graduating students
