To confirm your slot in the program, please accomplish the following:

Submit the following initial requirements to the Office of the Dean, College of Education, Talisay Campus at LSAB 114 on or before May 26, 2025
- Original Report Card for senior high school graduates, or transfer credentials with photocopy of Transcripts of Records for transferees.
- Certificate of Good Moral Character.
- Two (2) original copies of NSO/PSA Birth Certificate
- Two (2) original copies of NSO/PSA Marriage Certificate (additional requirement for married female applicant)
- Two (2) pcs recent 2×2 ID picture
- 1 pc long brown envelope with label at the back
Additional documents/requirements may be needed upon enrolment by the University Registrar. Also, applicants who did not qualify may retrieve their documents after May 30, 2025 at the College of Education office (LSAB 114).
