Congratulations to all the applicants of the Bachelor of Secondary Education major in Mathematics and major in Science, and Bachelor of Special Needs Education at the Talisay (Main) Campus who are now qualified for enrollment for Academic Year 2025-2026.
To confirm your slot in the program, please accomplish the following:
- Bachelor of Secondary Education major in Mathematics and major in Science (on or before May 20, 2025) https://forms.gle/7hQ2s5zCACtjiRoeA
- Bachelor of Special Needs Education (on or before May 20, 2025) https://forms.gle/Li77XSUTbdh2izMH6
- Fill out the Student Data Sheet in the link on or before May 20, 2025 https://forms.gle/uMyGVX7ECMYG9w5S8
- Submit the following initial requirements to the Office of the Dean, College of Education, Talisay Campus at LSAB 114 on or before May 20, 2025.
- Original Report Card for senior high school graduates, or transfer credentials with photocopy of Transcripts of Records for transferees.
- Certificate of Good Moral Character.
- Two (2) original copies of NSO/PSA Birth Certificate
- Two (2) original copies of NSO/PSA Marriage Certificate (additional requirement for married female applicant)
- Two (2) pcs recent 2×2 ID picture
- 1 pc long brown envelope with label at the back
Additional documents/requirements may be needed upon enrolment by the University Registrar.
Also, applicants who did not qualify may retrieve their documents after May 27, 2025 at the College of Education office (LSAB 114).
