Congratulations to the FIRST BATCH of successful applicants who are qualified for enrolment to the Bachelor of Elementary Education of the College of Education for 1st Semester of Academic Year 2025 – 2026.
To confirm your slot in the program, please accomplish the following:
1. Confirm your slot on or before May 26, 2025 by sending a text message with the following details to this number: 09085496099. Name:
Date of confirmation: Yes, I am enrolling in the BEED program of CHMSU.
Note: If you fail to confirm on the designated date, the slot will be forfeited.
2. Fill out the Student Data Sheet in the link on or before May 30, 2025 https://forms.gle/uMyGVX7ECMYG9w5S8
3. Submit the following initial requirements to the Office of the Dean, College of Education, Talisay Campus at LSAB 114 on or before May 30, 2025.
• Original Report Card for senior high school graduates, or transfer credentials with photocopy of Transcripts of Records for transferees.
• Certificate of Good Moral Character.
• Two (2) original copies of NSO/PSA Birth Certificate
• Two (2) original copies of NSO/PSA Marriage Certificate (additional requirement for married female applicant)
• Two (2) pcs recent 2×2 ID picture
• 1 pc long brown envelope with label at the back
BATCH 2 will be released next week together with the qualifiers included in the waiting list. Please watch out for it.
Note:
Additional documents/requirements may be needed upon enrolment by the University Registrar.
Also, applicants who did not qualify may retrieve their documents after the posting of BATCH 2 qualifiers at the College of Education office (LSAB 114).
