All CHMSU students from the Alijis, Binalbagan, Fortune Towne, and Talisay campuses who do not yet have a university ID are required to complete the Online ID Form for ID processing.
The form will be available from June 2 to June 10, 2026 at: cidsa.chmsu.edu.ph
Please ensure that:
- All male students follow the prescribed haircut policy.
- All female students comply with the university’s hair color policy.
Kindly complete the form within the specified period to avoid delays in ID processing.
