To confirm your slot in the program, please accomplish the following:
1. Fill out the confirmation slot in the link on or before June 15, 2026: https://forms.gle/jF8cVTzDdD7dLXGB6
2. Submit the following initial requirements to the Office of the Dean, College of Education, Talisay Campus, at LSAB 114 on or before June 18, 2026
- Original Report Card for senior high school graduates, or transfer credentials, with a photocopy of the Transcripts of Records for transferees.
- Certificate of Good Moral Character.
- Two (2) original copies of NSO/PSA Birth Certificate
- Two (2) original copies of NSO/PSA Marriage Certificate (additional requirement for married female applicant)
- Two (2) pcs recent 2×2 ID picture
- 1 pc long brown envelope with a label at the back
The University Registrar may require additional documents or requirements upon enrolment. Also, applicants who did not qualify may retrieve their documents after JUNE 18, 2026, at the College of Education office (LSAB 114).
