Congratulations to the successful applicants who are qualified for enrollment in the Bachelor of Secondary Education major in Mathematics of the College of Education for the 1st Semester of Academic Year 2026 – 2027.
To confirm your slot in the program, please accomplish the following:
- Fill out the confirmation slot in the link on or before June 22, 2026 https://forms.gle/7hQ2s5zCACtjiRoeA
- Submit the following initial requirements near the Faculty Office of the College of Education, Talisay Campus, at LSAB 122 on June 22, 2026, from 9:00 AM to 12:00 NN only.
- Original Report Card for senior high school graduates, or transfer credentials, with a photocopy of the Transcripts of Records for transferees.
- Clear copy of the Certificate of Good Moral Character.
- Clear copy of NSO/PSA Birth Certificate
- Clear copy of NSO/PSA Marriage Certificate (additional requirement for married female applicant)
- Two (2) pcs recent 2×2 ID picture
- 1 pc long brown envelope with a label on the back
The University Registrar may require additional documents or requirements upon enrollment.
Also, applicants who did not qualify may retrieve their documents after June 22, 2026, at the College of Education office (LSAB 114).
