Due to the system glitch, all applicants who reserved a slot for March 23, 2023 (original schedule) at Fortune Towne Campus will be rescheduled to the following dates: April 5, 11, 12, 13, 18, 19, and 20. Venues, date and time are indicated in the list.
Kindly bring the following:
- Duly Accomplished Application Form (CHMSUETAF)
- Learner’s Reference Number
- 2 pieces 2×2 ID Picture
- Any valid ID card
- 1 long brown envelope
- Pencil and eraser
- proof of your successful registration (screenshot)
Reminders: Be at your assigned testing room at least 30-45 minutes before the start of the test. Observe basic COVID-19 health and safety protocols.
