• Transfer Credential with photocopy of Transcript of Records for Transferees
•Certificate of Good Moral Character
• Admission slip and/or loading slip
• 2 copies of NSO/PSA Birth Certificate
• 2 copies of NSO/PSA Marriage Certificate for married female applicant
• 2 pcs recent 2×2 ID picture
• 1 long white/cream folder
• 1 long brown envelope
• Filled out Student Information Sheet (SIS)
• Php 130.00 Insurance Fee
Requirements for Enrollment for continuing students (Second Year to Fourth Year)
• Accomplished Clearance
• Loading slip for irregular students
• School ID
