To all 1st year and 2nd year students of CHMSC Talisay Campus
The ICT-MIS will print and issue identification (ID) cards (new and reconstructed) at the 3rd Floor, Green Building, Talisay Campus on the following dates:
March 23, 24, and 25, 2022 – 1st year students only
March 28, 29, and 30, 2022 – 2nd year students only
Please bring your completed enrollment form and COVID-19 vaccination card.
The schedule for 3rd to 5th year students of Talisay Campus and 1st to 4th year students of the other campuses will be released soon.
