After confirming your slot, kindly process the following requirements needed for enrollment
- Original Report Card for Senior High School Graduates
- Certificate of Rating (COR) for ALS Completers
- Transfer Credential with photocopy of Transcript of Records for Transferees
- Certificate of Good Moral Character
- Admission Slip and/or Loading Slip
- 2 copies of NSO/PSA Birth Certificate
- 2 copies of NSO/PSA Marriage Certificate for married female applicant
- 2 pcs recent 2×2 ID Picture
- 1 long white/cream folder
- 1 long brown envelope
- Filled out Student Information Sheet (SIS)
- Php 130.00 Insurance Fee
